Please complete this form to enter your suggestion for an additional item to be purchased by Skokie Public Library and added to the Library's collection.  For the request to be processed, your name and Skokie Public Library card barcode number must be entered.

If you would like the item to be reserved for you once received by the Library, you must also provide your mailing address.  You will be notified by mail if the Library does not purchase the requested item.
